GENERAL INFORMATION

The Cancer Association of Champaign County (CACC), is a non-profit charitable organization existing to assist cancer patients of Champaign County in defraying the cost of drugs, treatments, prosthesis and other services and expenses associated with their care.  In the interest of serving as many patients as we can simultaneously, certain guidelines have been developed concerning this financial aid.

It is recognized that sometimes certain occasions and circumstances will arise where there are special needs which do not fall within the stated Association guidelines.  Such requests, will be addressed on a case by case basis and will require board review before any funding is authorized.

Financial assistance is currently limited to a maximum of $300.00 per month, per patient.  This is subject to review by the CACC board of directors, dependent upon the financial position and monthly demands of the Association.  However, expenses other than prescriptions and personal medical supplies in any instance will not exceed $100.00 per month, per patient. - top

 

PRESCRIPTIONS

Cancer related (CR) prescription balances will be paid after all other patient coverage's (insurance, Medicare, etc.) have been exhausted - up to the $300.00 maximum benefit.  Drugs on the CACC list of Non-Cancer related drugs will not be paid, unless that prescription has been marked "CR" by the attending doctor.  Colostomy supplies, oxygen and other personal items will be paid in the same manner.  CACC will also assist in the rental payment of wheel chairs, hospital beds, and other equipment needed by the patient.  The total of all expenses cannot exceed the maximum benefit.  Chemotherapy and radiation treatments will be covered as prescriptions and will follow the above guidelines. - top

 

PROSTHESIS

CACC will assist with necessary prosthesis at the following rates: Wigs/Toupees - $100.00.  Bras - $200.00 with a limit of 2 per year. - top

 

DOCTORS and HOSPITAL TREATMENTS

CACC will assist patients with bills for treatments received at their doctors office or at the hospital.  This does not include routine office calls (follow-ups or check-ups) to the doctors office or hospital.  Patients must submit an itemized statement from the provider, showing all charges and credits, and the balance due after payments received from all insurance companies and other entitlements are credited.  If more than one bill submitted for consideration, it is the patients responsibility for setting the priority of payments.  Maximum benefit may not exceed $100.00 of the monthly maximum and will be distributed after the patients monthly prescriptions have been received. - top

 

GASOLINE

CACC will assist patients in the purchase of gas for the transportation of patients receiving treatments at a physicians office or hospital.  Agreements have been set up with Bell’s BP CAR CARE on Scioto Street in Urbana, Certified Gas in Mechanicsburg, St. Paris Oil Company in St. Paris, and Packman’s IGA in North Lewisburg to allow patients to charge their gas to CACC.  The following amounts of purchase are authorized:  Trips to Springfield: patients may charge up to $5.00 of gas per day.  Trips to Columbus or Dayton: patients may charge up to $7.50 of gas per day.  A maximum of $7.50 per day may be charged (i.e. you may not charge $5.00 for a trip to Springfield and then charge $7.50 for a trip to Dayton or Columbus on the same day).  A copy of the attached form (SAMPLE 1) must be submitted monthly by the patient.  This form provides a “proof “ of service needed for auditing purposes.  Failure to file this form monthly may result in loss of gasoline benefit.  This form shall include dates of treatments / visits, and must be signed by the doctor or a representative of the office.  Forms are available at the gas stations. - top

 

OTHER GUIDELINES

Patients may choose the pharmacy of their choice, (except REVCO or any other pharmacy which may not be participating with CACC). 

CACC will make payments only to medical providers for cancer related expenses.  No direct reimbursement will be made to individual patients without the consent and approval of the CACC Board of Directors.

CACC will not be responsible for bills existing 30 days prior to acceptance as a patient of CACC.

CACC will not pay in-patient hospital bills except as provided under the section titled “DOCTORS AND HOSPITAL TREATMENTS”.

CACC reserves the right to modify monthly payment schedules based on current demand and financial position of the Association. - top
